Rates And Insurance

RATES

 Therapy Sessions (Individual, Couple and Family)              

$175 for Initial Intake Session              

$150 per session (50 minutes)     

In-Home Sessions              

$150 per session (50 minutes) plus gas and travel time     

Telephone Sessions              

$20 per 15 minutes     

Clinical Supervision              

$100 per Individual Supervision (1 person)              

$75 per Individual Supervision (2 people)              

$50 per Group Supervision (3 or more people)     

*Documentation preparation, assessments, court and related appearances require additional fees*

INSURANCE

  I am a provider for some insurance panels and as an out-of-network provider for most of the major insurance companies.  This means that if you have an out-of-network plan your insurance provider may pay for a percentage of my fee after your annual deductible is met.  Services may be covered in full or in part by your health insurance or employment benefit plan.  Upon request, I will fill out insurance forms or provide assistance to expedite reimbursement.  Please check your coverage carefully by asking the following questions to your insurance carrier:

  Do I have mental health benefits?              

  What is my annual deductible and has it been met?              

  How many sessions per calendar year does my plan cover? 

  How much does my plan cover for an Out-of Network provider? 

  What is the coverage amount per therapy session?             

   Is approval required from my primary care physician?              

   Is there a need for prior authorization?  

Most insurance companies require me to provide them with a clinical diagnosis.  In some cases I might have to submit treatment plans and summaries.  This information would become part of the insurance company files.  Although insurers insist that all records are kept confidential, I am unable to guarantee the privacy of any information after it leaves my office.

REDUCED FEE

 Reduced fee services are available on a limited basis.    

 PAYMENT

 Cash and check accepted for payment.     

CANCELLATION POLICY

 24 hours in advance notice is required for cancelling a scheduled appointment.  You will be required to pay the full cost of the treatment as booked, when advance notice is not provided.  Emergencies are understood and certainly taken into consideration.
